Person-Centered Planning
We provide Person-Centered Planning and coordination of services as an independent agent. That means we only provide case management. No residential supports or direct support services. Other organizations provide case management and direct services that can be a conflict and limits choice. We assist in connecting individuals and families to community resources and coordinating multiple services. We monitor services to make sure individuals get what they need, when they need.
Advocacy
We take time to get to know each individual so we can support their wants, needs, desires and dreams. Instead of “we can’t”, we ask, “how can we make that happen?”
We believe each individual we serve has the right to:

We believe each individual we serve has the right to:
- Have choice in how they live their lives, where and with whom.
- Relationships that provide the strength, assistance, and security to better the individual’s well-being.
- Contribute to their community in a meaningful way.
- Have an active role in their team by communicating their wants, needs and desires while also being responsible for their own actions and decisions.
- Be in the driver’s seat. To decide what services to receive and who should provide them. This includes hiring and firing staff that do not work well with you.
- Dream. We respect your dreams and will help you make them come true.
- Dignity and respect by everyone involved in your team.
